What the fees don't cover

Evaluation and membership fees do not include the cost of medication itself. Prescriptions are filled at your pharmacy and paid there. Your prescription drug coverage works normally even though we don't bill insurance for visits.

Insurance and superbills

We don't bill insurance directly. You pay at the time of service and we provide a superbill on request: a detailed receipt with the diagnosis codes, procedure codes and provider information your insurance company needs. You submit it to your insurer and they reimburse you based on your out-of-network benefits. Reimbursement varies by plan. To get a sense of what to expect, call the number on your insurance card and ask about your out-of-network mental health benefits, specifically your deductible, coinsurance and any session limits.

If your plan requires a prior authorization for a medication, we handle it. Most decisions come back within a few business days, and if you'd rather not wait we can usually start you on a well-covered option right away and switch once the authorization clears. Either way we'll point you to manufacturer coupons and other ways to bring the cost down.

Re-evaluation pricing

If the answer is not ADHD today and the issue we named is addressed later, a re-evaluation within 12 months is $395. If the picture is unclear at the end of your visit, one additional evaluation visit within 90 days is included at no charge.

Cancellation and refunds

Cancellation and no-show policy

Appointments require at least 48 hours' notice to cancel or reschedule. Late cancellations (less than 48 hours) and no-shows are charged a $150 fee. This fee is not billable to insurance and is not eligible for superbill inclusion. The fee applies to all appointment types, including membership follow-up visits.

What happens to the $695 evaluation fee: if you cancel or reschedule with at least 48 hours' notice, it is refunded in full or credited to a rebooked visit, your choice. If you cancel with less than 48 hours' notice or do not attend, the $150 fee applies and the $695 is credited to one rebooked visit within 60 days; if you do not rebook, it is refunded less the $150 fee. If your intake paperwork or cognitive testing, including testing you started but did not finish, is not complete by 11:59 pm Central the day before your evaluation, or by a revised cutoff we have given you in writing, the visit is rescheduled and the $695 is credited to the rescheduled visit; the $150 fee is waived for a first occurrence. If a delay on our side, such as a late testing link, or a failure of the video or testing platform is the reason, nothing is charged.

Joining more than 15 minutes after your appointment time is treated as a missed appointment under this policy unless Dr. Bracciano chooses to proceed, in which case a late start may shorten the visit. Tell us as early as you can if you will be late or cannot attend; documented emergencies are handled at the practice's discretion.

Membership cancellation

You may cancel your ongoing care membership at any time by emailing office@exhalepsych.com or by writing to Exhale Psychiatry, S.C., 9000 West Chester Street, Suite 100, Milwaukee, WI 53214. Your paid month is the monthly cycle that begins on your charge date. Your cancellation counts from the day we receive your email or letter, whether or not you have seen our confirmation; if no confirmation arrives within one business day, contact us again. Cancellation takes effect at the end of the month you have already paid for: you keep access through that paid period, there is no further charge and no partial-month refund. If we end your care, the unused portion of the current month is refunded pro rata. Active prescriptions are managed through a transition period to support continuity of care.

Non-refundable evaluation

The evaluation is complete when the visit has been held and the written report delivered; once complete, the evaluation fee is non-refundable regardless of the diagnostic outcome. An evaluation interrupted before completion resumes at no new charge. If a different condition is identified, we explain the findings, include them in your report and refer you to the right provider.

These terms come from the Financial Agreement you sign before your visit. The agreement is the full policy and also lists the situations where no fee applies.

Paying with an HSA or FSA

We accept credit cards, debit cards and HSA/FSA cards. Whether your HSA or FSA covers evaluation or membership fees is decided by your plan administrator. The $150 late cancellation and no-show fee is never charged to an HSA or FSA card.
